Our healthcare systems are not centralized....yeah some hospitals are better and have better resources, and patients get transported around if the situation calls for it. Sometimes the best care you'll get, you'll get in a rural hospital. But no one waits for months for a diagnosis here(unless its something wacked and it stumps doctors), you wouldnt wait months for dialysis or chemo. You wouldnt wait weeks to get surgery to repair a break(while risking a host of complications directly caused by waiting)
